Refined foods typically refer to grain products.

When a grain is refined, the nutritious bran, germ and husk layers are removed, leaving only the endosperm–or inner part of the grain.  Without the bran and germ layers, most of the fiber is also stripped out, increasing the glycemic index of the resulting food.
